Membership Requirements:
A
membership as an Affiliate Member or Facility Member or Facility
Member may only be issued to a Person who is a freestanding ambulatory
surgical center facility located within the State of Idaho or in
a county in an adjoining state whose county line coincides with
the state line of the State of Idaho.
An ambulatory surgical center is defined as a facility that is dedicated
exclusively for the purpose of providing surgical and anesthetic
services to patients. As such the ambulatory surgical center must
meet conditions deemed appropriate relative to the scope of ambulatory
surgical and anesthetic services so provided. In addition, all Facility
Member ambulatory surgical centers must be certified by the Idaho
Department of Health and Welfare or Medicare and/ or be accredited
by JCAHO/AAAHC.
No
ambulatory surgical center affiliated with a hospital owning a controlling
interest in such ambulatory surgical center can acquire a Membership.
A Controlling interest is defined as fifty-one (51%). In addition,
no ambulatory surgical center affiliated with a hospital owning
less than a controlling interest in such ambulatory surgical center
can acquire a Membership unless such ambulatory surgical center
is held as a separate and distinct Entity, with all required certification
also held separately and distinctly form any hospital certification.
